Employees were tracing a line of a transformer to conduct repair after the transformer had been struck by lightning. The transformer was for an irrigation machine. Employees dug a ditch and found that there was another primary line which was damaged and not in use. The injured employee was checking this line with a low voltage meter. When he checked the second phase, an arc flash occurred. The employee was hospitalized with burns to his left hand.

On 9/06/2020, a spud barge with an excavator was tied to a deck barge by two steel cables around timber heads, one forward and one aft. As the excavator was removing rock from the deck barge, the deck barge began to sway, creating more tension on the aft cable. At 1:00 PM, the cable slipped over a timber head on the spud barge, released, and struck an employee on the spud barge and propelled them overboard. The employee was hospitalized with fractures to the left hand.

An employee was working on a rail car ladder, 8-10 feet off the ground, putting a tarp on the rail car. A strong gust of wind blew the tarp, causing the employee to lose his footing. He fell from the ladder to the ground, breaking his right leg and right elbow.
